How it works

Active patients can request a referral card from a member of our team. We will sign the card and give it to you.

You take the card to a friend or family member you believe would benefit from the services we offer at Rockford Wellness Collective.

Once your friend schedules an appointment with us and brings in their referral card, the referring and referred patients will receive a $30 credit to their account! *Terms and conditions apply*
Terms & Conditions
Rockford Wellness Collective appreciates your referrals! Our referral program is designed to reward our valued patients for referring friends to experience our services. To participate, existing patients need to refer a friend who is not currently a patient at Rockford Wellness Collective. When the referred friend purchases one of our standard services, the existing patient and the referred friend will receive a $30 credit. This credit has no cash value and will be applied as a credit to the respective patient's account to be used for packages or individual sessions.

The credit must be redeemed within one year from the date of issuance. Each patient is eligible for a maximum of 5 referrals per calendar year. Please note that the credit cannot be used for initial consultations or supplement purchases. Additionally, the referral program credit cannot be combined with any other offers, discounts, or promotions unless explicitly stated by Rockford Wellness Collective. We reserve the right to modify or terminate the referral program at any time without prior notice. We appreciate your continued support and participation in our referral program
